Phothera home phototherapy is available through three submission options:

  1. EMA®, by Modernizing Medicine®. Preferred. Prescribe directly from the patient chart. PhotheraHCP.com/prescribe/emr-integrations
  2. Phothera e-form. PhotheraHCP.com/prescribe/e-form/
  3. Fax form. PhotheraHCP.com/prescribe/#fax
